Our founders — parents, and carers firstWhy we exist
Over a decade ago our own child was diagnosed with disability. We know what it is to be handed a plan you don’t understand, in a system nobody explains, while trying to make sure your child gets what they need.
Coming from a culturally diverse background, one of our hardest battles was acceptance — in our community, and in systems that were never designed with our family in mind. Those years taught us exactly what good support looks like, and exactly what it doesn’t.
For us, this is more than a profession. It is personal.
Coordinate & Care exists so that no family has to feel alone, unheard or unsupported. It shapes how we answer the phone, how we write, who we hire, and how hard we push when something isn’t right.

No sales pitch and no forms to decode. Four steps, and you can stop at any of them.
You call, email or send the referral form. We listen — properly — to what is happening right now and what you want to change. No jargon, no rush, and no obligation.
We meet you where you are comfortable, go through your NDIS plan together in plain language, and map out your goals, your supports and the gaps between them.
We find and connect the right providers, set up agreements and bookings, brief everyone involved, and make sure supports actually start when they are meant to.
We check in, adjust what is not working, document your progress, and prepare properly for your plan review — while building your confidence to direct more of it yourself.

Common questions
If your question isn’t here, call us. We answer these all day and there’s no such thing as a silly one.
Call us. We will talk you through the access process, what evidence you are likely to need, and what to expect from a planning meeting. We do this at no cost and with no obligation — helping families understand the system is the reason we exist.
For coordination, yes — it is a specific line item in your plan. If it is not funded and you think you need it, we can help you build the case for it at your next review. Many of our other supports are funded from core or capacity building budgets instead.
Yes, and it is more straightforward than most people expect. You are free to change providers at any time. We handle the handover, request your existing documentation, and make sure nothing is dropped in the transition.
We work with all three. Tell us how your plan is managed and we will set up the service agreement and invoicing to match, then explain exactly how the money flows so there are no surprises.
