Why we exist

We were the family sitting on the other side of the desk

Over a decade ago our own child was diagnosed with disability. We know what it is to be handed a plan you don’t understand, in a system nobody explains, while trying to make sure your child gets what they need.

Coming from a culturally diverse background, one of our hardest battles was acceptance — in our community, and in systems that were never designed with our family in mind. Those years taught us exactly what good support looks like, and exactly what it doesn’t.

For us, this is more than a profession. It is personal.

Coordinate & Care exists so that no family has to feel alone, unheard or unsupported. It shapes how we answer the phone, how we write, who we hire, and how hard we push when something isn’t right.

What working with us actually looks like

    First conversation

    You call, email or send the referral form. We listen — properly — to what is happening right now and what you want to change. No jargon, no rush, and no obligation.

    Getting to know you

    We meet you where you are comfortable, go through your NDIS plan together in plain language, and map out your goals, your supports and the gaps between them.

    In your home, online, or somewhere neutral

  3. 03

    Putting the plan into action

    We find and connect the right providers, set up agreements and bookings, brief everyone involved, and make sure supports actually start when they are meant to.

    You approve every provider before we proceed

    Reviewing and growing

    We check in, adjust what is not working, document your progress, and prepare properly for your plan review — while building your confidence to direct more of it yourself.
