Specialist Support Coordination

When a situation is complex, high-risk or stuck, it needs a coordinator with the seniority and experience to move it.

Specialist Support Coordination is a higher level of support for participants whose circumstances involve significant risk, multiple interacting services, or barriers that ordinary coordination cannot shift on its own.

We map out everything that is happening, identify what is creating the risk, and build a plan that brings health, housing, justice, education and disability services into the same conversation. We chase the follow-ups, document what happens, and escalate when we need to.

This is hands-on, senior work. It often involves hospital discharge planning, housing breakdowns, restrictive practice reduction, or supporting someone whose supports have collapsed and need rebuilding from the ground up.
