Transitions are where plans most often fall apart. They are also where good support makes the biggest difference.
Big changes bring new systems, new people and new paperwork all at once. We work ahead of the change: mapping what needs to happen, who is responsible, and what has to be in place before the date arrives.
We stay on after the transition too, because the first few weeks in a new situation are usually when problems surface.
